This is patch 5 for Proforms version 1.3 - build 111

Patch 5 does also include Patch 1,2,3 and 4 which means you don't need to apply the former patches additionally.
DON'T APPLY PATCH 1,2,3 OR 4 AFTER PATCH 5 THIS WILL OVERWRITE ALREADY IMPORVED FILES!

Patch 5 corrects the tooltip bug on chrome browsers (wrong position) and the SQL options bug for selection fields.
           (SQL queries had been escaped before queried which lead to malfunction)
Patch 4 corrects the app function. The app function did show only one form template's aliases / fields.
Patch 3 corrects the copy function of forms which had a bug in the SEF URL generation.
Patch 2 corrects the scroll to confirm app bug and applies a better structure and view for HTML Emails.
The including parts of patch 1 are for some tiny bugs and stability patch.
